A Study on Patterns of Pain Reaction according to the Time Since Surgery


Abstract
The purposes of this study were
1) to explore how pain reaction would change according to the time since surgery.
2) to identify the relationships of pain reaction with the independent variables of patients such as sex, age, previous experience of surgery -with the control of the time since surgery.
Twenty subjects with the age range of 18 through 50 were collected at three general surgery departments in Seoul National University Hospital.
Each subject participated in the experiment from 1 to 4 times until 49 hours since surgelry to reach total 53 times during the period from August 16 to September 20, 1982. f 1
OPRRS (Overt Pain Reaction Rating Scale) designed by Locsin was translated in korean and used as a study tool for measurement of pain reaction.
Kendall¢¥s coefficient of concordance and Cronbach¢¥s alpha were used to test the objectivity and reliability of the pain measurement tool. ANCOVA (Analysis of covariance) and Scheffe test (Multiple range test) were used to test specific hypotheses. The results of this study were summarized as follows;
1) kendall¢¥s coefficients of concordance were from 0. 60 to 0. 85 according to items. Reliability coefficient using Cronbach¢¥s alpha was 0.93.
2) There was a significant decrease in pain reaction according to the time since surgery (P=O. 0002). That is, pain reaction during first 12 hours since surgery was significantly higher than that during the period of 24 to 49 hours since surgery.
3) There was a significant main effect at 0. 013 level in pain reaction by three independent variables when the time since surgery was partialled out.
There were significant differences in sex at 0. 05 level and in age at 0. 035 level; but no significant difference in previous experience of surgery. Pain reactions of the male subjects (20.15¡¾6.25) were higher than those of the female subjects (16.85¡¾5.05).
As a whole, scores of Pain rating were low and medians of responses were also low degree.
Since this pain rating was done by observation of patient¢¥s physical activities and physiol ogical responses, it would be different from the ratings obtained by patient¢¥s self-report.
A repeat study using this OPRRS is necessary in the future study.
